OneCare Launches Innovative Adherence Management System

MagnaCare Signs On as First Major Healthcare Customer
MARIETTA, GA – OneCare’s groundbreaking new system for medical case management is being rolled out to chronically ill patients by MagnaCare, a Brighton Health Plan Solutions business with more than 500,000 covered lives in the Northeast.

  • OneCare, based in metro Atlanta, combines cutting-edge technology with social media, behavioral change methods and family-based communication strategies to revolutionize case management for the chronically ill. The cloud-based, HIPAA-compliant OneCare system:
  • Provides current and accurate information on chronically ill and high claims risk members without relying on patient self-recording.
  • Utilizes social media and mobile apps to involve family members in day-to-day case management.

OneCare Selected as Care Management Platform for InHealth Mutual of Ohio

With the announcement of another major collaboration project, OneCare is once again proving itself as a leader in providing effective connected health solutions with its innovative care management platform. The OneCare platform has been chosen by InHealth Mutual of Ohio to host and implement several innovative care management programs, beginning with a Depression Management program launching in November.

The OneCare Platform was chosen to deliver a fully encompassing program that provides education, assessments, mood logs, journals, as well as diet, exercise and medication tracking through mobile and web-based user experiences. Throughout each week of the program, multiple lifestyle factors are logged to help identify trends in user behavior as it correlates to depression. Enrolled participants are encouraged to engage a local support team of family and friends as part of the program as well as their primary care providers and other professional care givers. Providing secure messaging and data sharing as well as alerts and reminders via email, text message or voicemail, the Platform enables InHealth to effectively manage care collaboration amongst the local support team, professional care team, and in-house disease educations.

Another important facet of the Depression Management Program is medication management. Participants can input any medications they are currently prescribed (or have been in the past) as well dosage information and instructions. Schedules are then created, allowing participants to receive medication reminders as well as track compliance along with any side effects.
